Work Environment\n

This position will be executed remotely, fostering a flexible and dynamic work setting. As a Home Care Social Worker, you will primarily engage in virtual consultations and documentation while coordinating home visits. Your work hours are typically Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend availability to respond to patient needs. Flexibility in scheduling is crucial to ensure that clients receive consistent, quality care.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, evaluating emotional, mental, and social factors to develop personalized care strategies.

• Create individualized care plans that reflect the goals and preferences of clients and their families, incorporating feedback and encouraging active participation.

• Offer counseling and emotional support, guiding seniors and their families through life transitions such as health changes, loss, or increased dependency.

• Advocate for clients, ensuring they have access to community resources and services that meet their unique needs, including health care and social support.

• Collaborate with an interdisciplinary team, including nurses and caregivers, to synchronize care efforts and address any client concerns effectively.

• Regularly conduct home visits to monitor clients’ conditions, assess living environments, and identify areas for improvement or concern.

• Facilitate open communication within families, helping resolve conflicts to create a supportive network around the senior client.

Qualifications\n

• Bachelor's Degree in Social Work from an accredited institution.

• Current and valid state licensure as a Social Worker (LCSW, LMSW, or equivalent).

Compensation and Benefits\n

The annual salary for this position ranges from USD 60,000 to 100,000, commensurate with experience and qualifications.
