Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify the needs and concerns of seniors, integrating emotional, social, and mental health factors.

• Develop, implement, and monitor personalized care plans in collaboration with clients and families, adapting to changes in their circumstances or health.

• Provide individual and family counseling to assist clients in managing transitions, coping with stressors, and improving their overall emotional well-being.

• Advocate for clients to gain access to necessary community resources, including healthcare services, social programs, and support networks.

• Conduct regular virtual check-ins to assess client progress and well-being, ensuring quality care delivery and timely intervention as needed.

• Educate clients and their families about available community resources, fostering engagement and support in their journey toward improved health and well-being.

• Document assessments, care plans, and client interactions diligently within our electronic health record (EHR) system, ensuring compliance with ethical and legal standards.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in California.

Compensation and Benefits\n

The annual salary for this position ranges from USD 100,000 to 120,000, commensurate with experience and qualifications.
