Work Environment\n

This position offers the flexibility of remote work, allowing you to engage with clients from the comfort of your home while providing essential support to seniors in their living environments. Your typical work hours will be Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours necessary based on client needs. You will have the autonomy to schedule visits and ensure that you are available for your clients, adapting your schedule as needed to provide continuous care. Being on call to manage urgent situations will also be part of your responsibilities, ensuring client needs are met promptly.

Key Responsibilities\n

• Conduct comprehensive assessments of the psychosocial needs of senior clients, focusing on emotional, mental, and social factors that impact their daily lives.

• Collaborate with clients and their families to create personalized care plans that align with each individual's specific goals and preferences.

• Provide counseling and emotional support to seniors and their families, assisting them in navigating transitions such as bereavement, relocation, or health changes.

• Advocate for clients to ensure their rights are respected and that they have access to necessary community resources and services.

• Work as part of an interdisciplinary team, engaging with nurses, caregivers, and other healthcare professionals to coordinate comprehensive care.

• Perform regular monitoring visits to evaluate client well-being, assess living conditions, and address any safety or health concerns.

• Facilitate effective communication and conflict resolution among family members involved in the care process.

Qualifications\n
